This is a macroscript, to install it extract this file in the UI\macroScripts folder of your 3ds max root directory, restart max, then go to Customize->Customize interface, in the category Custom tools select TwistBones and assign it a shortcut or a toolbar button or a menu place or a quat place. Once executed a dialog box is displayed where you can setup the rig.
This tool is useful to create twistbones (TBs) for all the limbs of the cs biped . The adventage of these TBs is that they dont flip unless the limb overlaps its parent. The utility creates a main TB with a rotation_script controller and several extra TBs with orientation constraints from the main TB to the respective limb, with weights regards to their position in the TBs hierarchy.
The UpperArm, Forearm, Thighs and Calves spinners control the ammount of twist bones per limb respectively. Select the bipeds that you want to set the twistbones in and press Set TwistBones (You can select any part of the biped).
